How to Apply
Four Simple Steps

Download the application packet
Get the 2026–2027 Admissions Application below. Please read, sign or initial, and date each page.

Gather the required documents
Use the checklist on this page to collect everything the school needs — including your child's last report card, which must be presented when the application is submitted.

Bring your packet to the school office
Submit the completed application and documents in person at 1222 Riverside Blvd, Memphis, TN 38106. Questions? Call (901) 775-0304.

Attend Parent/Teacher Orientation
Once your child has been accepted, the final step in the enrollment process is a Parent/Teacher Orientation (date to be announced).
For your family's privacy, we accept completed applications in person only — the packet contains sensitive information that should not be sent by email. Our staff will be glad to receive it at the front office.

A completed application must be submitted along with the following documents. If further information is needed, our administrative staff will contact you.

- ✓Shot Records (double dose of MMR for kindergartners, 4th, and 8th graders, or a tentative date)
- ✓Social Security Card
- ✓Insurance Card
- ✓Proof of Residence (2 forms)
- ✓Proof of Income (2 forms)
- ✓Form 1040 Income Tax Return
- ✓Last Report Card (must be presented when the application is submitted)
- ✓Standardized Test Scores
- ✓Authorized Pick-Up and Drop-Off List
- ✓Transcript Release Form
- ✓Standards of Safety
- ✓Transportation Agreement (if applicable)
